29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: On June 10, 2015, the employer did not evaluate the respiratory hazards, including a reasonable estimate of employee exposure to hydrogen cyanide during installation of a blind into process piping at the Heads Column, DA-0501 at the Ineos Nitrile, Green Lake facility. Where a reasonable estimate of an employee exposure to hydrogen cyanide could not be made; the employer did not consider the hydrogen cyanide atmosphere to be immediately dangerous to life and health.
