29 CFR 1910.27(f): Ladders were not maintained in a safe condition or inspected regularly: a) Rough Mill, Back Room, on or about 4/15/15: A 20 foot fixed ladder for access to the roof was not maintained in a safe condition in that the side rails were severely bent and two rungs were missing at the bottom of the ladder. Abatement certification must be submitted for this item.
