OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to anhydrous ammonia inhalation hazards: On or about March 3, 2015, in the Engine Room and at the Medium Receiver Vessel room, oil removal fixtures for oil pots were not equipped with self-closing or quick closing valves at the oil drain points as required by the American National Standards Institute/International Institute of Ammonia Refrigeration, ANSI/IIAR-2 Equipment, Design, and Installation of Ammonia Mechanical Refrigerating Systems to prevent inadvertent or uncontrollable release of ammonia following oil drainage. Among other methods, a feasible and acceptable means of abatement include, but are not limited to the following: 1. Installation of dead man handle valves at oil pot drain points.
