29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: On or about March 2, 2015, upper mill area, the floor had numerous holes around the circular saw. The holes were measured at 4 inches in diameter. Employees are exposed to the hazard of tripping in the holes and falling.
