29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide alongside the dock wall. On or about 01/27/2015, at the above addressed worksite, an employee working without fall protection was exposed to a fall hazard onto steel beams while working on a narrow 4-inch plank alongside a dock wall.
