29 CFR 1910.146(k)(3): To facilitate non-entry rescue, retrieval systems or methods shall be used whenever an authorized entrant enters a permit space, unless the retrieval equipment would increase the overall risk of entry or would not contribute to the rescue of the entrant: At a facility located at 4315 Transport Way in Weston, WI; the employer did not ensure employees utilized retrieval systems or methods for non-entry retrieval of an entrant in the permit-required confined space of truck tanker trailers when employees entered and conducted cleaning the permit spaces on or about April 13, 2023, April 17, 2020, May 1, 2023 and June 12, 2023.
