29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about May 16,2023, in Department #7 Screw Machine Building, a lockout tagout device was affixed to a Tsugami Swiss Machine by an employee who was not authorized. This condition exposed employees to crush-by, struck-by, impalements, third and fourth degree burns and/or electrical shock hazards.
