29 CFR 1910.219(b)(1): Flywheel(s) with parts seven feet or less above floor(s) or platform(s) were not guarded in accordance with the requirements specified in 29 CFR 1910.219(b)(1)(i) through (b)(1)(iv)(a): (a) Perry Tool & Die Co. at 11606 Reed Ct., Broomfield, CO 80020: On and before June 23, 2017 the employer did not ensure that the flywheels, which were approximately 4 feet above the shop floor, on the Perkins Machine Company mechanical punch press model 300-B, were adequately guarded, in that only the belt section of the flywheel was guarded. This condition exposed employees to in-going nip points and amputation hazards.
