29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry: 194 Cleveland St. Greenfield, MA: On or about January 24, 2025, the employer did not implement procedures for either summoning or safely conducting rescue operations for entrants into permit spaces, nor did they prevent the unauthorized entry of two employees attempting to rescue an engulfed employee in a permit space, exposing them to engulfment and asphyxiation hazards.
