29 CFR 1910.146(k)(2)(iv): Employees were not provided practice permit space rescues at least once every 12 months, by means of simulated rescue operations in which they remove dummies, manikins, or actual persons from the actual permit spaces or from representative permit spaces: a. On or about November 9, 2016, at the above addressed location, employees were required to perform rescue duties for employees in permit required confined spaces. Employees were exposed to hazards associated with permit required confined spaces when rescuers were not provided training which simulated actual rescue with dummies, manikins and/or persons on at least an annual basis.
