29 CFR 1910.177(g)(8): The employer did not ensure that employees stay out of the trajectory when inflating a tire on a single piece rim wheel: The employer does not ensure employees remain outside the trajectory when they are inflating tires. This violation most recently occurred on or about April 3, 2014, at the establishment located at 1902 South Park Avenue in Stuttgart, Arkansas, where employee stood within the trajectory while seating and inflating a 28" Dee Stone tire to a single-piece rim wheel and the tire exploded. This practice resulted in the employee being struck by the exploding tire which caused fatal injuries.
