29 CFR 1910.332(b)(1):Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: a.) Ellwood City Forge Company; 800 Commercial Avenue Ellwood City, PA 16117, on or about 08/24/2025, and times prior thereto: Employees were not able to decipher the need to refer to NFPA70E table 130.7(c)(15) to select the appropriate level of arc-rated clothing upon entering the working distance (36-inches) as identified on the T2/T3 arc flash label while working inside the energized General Electric 4160 volts T2/T3 switchgear in the Millwright Shop Room. BATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
