29 CFR 1910.134(k)(1): The employer did not provide effective training that covered the required elements in 1910.134(k)(1)(i) - (vii): Forestar Industries located at a worksite at 2221 North Belcrest, Springfield, MO 65806: Employees were exposed to respiratory hazards. This was demonstrated on or about January 13, 2021, and at times prior thereto, the employer did not provide employees with effective comprehensive training that included but was not limited to how improper fit, usage, or maintenance can compromise the protective effect of the respirator, the limitations, and capabilities of the respirator(s), maintenance and storage of the respirator(s), how to use the respirator effectively in emergency situations, including situations in which the respirator malfunctions, and how to recognize medical signs and symptoms that may limit or prevent the effective use of a respirator(s). The employer required employees to wear respirators throughout their shifts as a requirement of their job duties during certain operations in the facility.
