29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter: (a) On or about January 10, 2019 and at times prior thereto, employees required to wear half-faced respirators while mixing chemicals in the mixing area were not fit tested prior to the initial use of the respirator, exposing employees to respiratory hazards from Amvos Restech, Diisonony Pthalate (DNIP), Chlorpyrifos, Deltamethrin, K-Bin Dryblend and Granular Absorbent.
