29 CFR 1910.272(g)(4): The employer did not provide equipment for rescue operations, which was specifically suited for the bin, silo, or tank being entered: (a) MEYERS GRAIN FACILITY, BIN 4 - The employer did not ensure that rescue equipment was available while employee(s) had entered the bin to clean the corn and gluten feed pellets that had accumulated on the sides, on or about October 15, 2018. (b) MEYERS GRAIN FACILITY, BIN 6 - The employer did not ensure that rescue equipment was available while employee(s) had entered the bin to clean the dried distiller grain that had accumulated on the sides, on or about October 15, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
