29 CFR 1910.146(d)(4)(ix): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not provide at no cost to employees any other equipment necessary for safe entry into and rescue from permit spaces: On or about October 7, 2014, the employer did not provide or ensure the use of necessary rescue equipment in the event of an emergency in a confined space. Employees were exposed to inhalation hazards.
