29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized, other safety related work practices were not used to protect employees who could be exposed to the electrical hazards involved: a.) Ellwood City Forge Company; 800 Commercial Avenue Ellwood City, PA 16117, on or about 08/24/2025, and times prior thereto: Employees were working inside the energized General Electric 4160 volts T2/T3 switchgear in the Millwright Shop Room. The line side of the 4160 volts switchgear was not de-energized or locked out and/or tagged out prior to performing work operations. Safety-related work practices including, but not limited to, the use of appropriate electrically rated personal protective equipment, were not utilized while working inside the established working distance inside the switchgear. b.) Ellwood City Forge Company; 800 Commercial Avenue Ellwood City, PA 16117, on or about 08/24/2025, and times prior thereto: Employees were working inside the energized General Electric 4160 volts T2/T3 switchgear in the Millwright Shop Room. The 4160 volts switchgear was not properly de-energized or locked out and/or tagged out at the lever prior to performing work operations. Safety-related work practices including, but not limited to, the use of appropriate electrically rated personal protective equipment, were not utilized while working inside the established working distance inside the switchgear. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
