29 CFR 1910.184(i)(9)(iii): Synthetic web sling(s) with snags, punctures, tears, or cuts were not immediately removed from service: On or about October 7, 2024, it was documented that emplo yees engaged in pretensioning activities at Coreslab Structures (Omaha) Inc. located at 802 Alliance Road, Bellevue, Nebraska were exposed to struck-by hazards in that employees manually braced and/or manipulated hydraulic jacking rams suspended by damaged synthetic web slings from a crane while the jacking ram was being operated. Instances included, but were not limited to the synthetic web slings used with: i. Daily operations in building 1 where employees used the hydraulic jacking ram; G. T. Bynum CO. Model M P U28, Serial number: 122297 and ii. Daily operations in building 1 where employees used the hydraulic jacking ram; G. T. Bynum CO., Model: M P U2, Serial number: 111396.
