29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about August 15, 2024, and at times prior, the employer had not developed and utilized hazardous energy control procedures for Magna Barrel Mixer to isolate the electrical energy source. This exposed employee's hands and arms to the rotating barrel's caught-in hazard, likely to result in serious crushing and amputation injuries when cleaning/sanitizing the equipment.
