29 CFR 1910.147(c)(7)(iii)(B): Additional retraining shall also be conducted whenever a periodic inspection under paragraph (c)(6) of this section reveals, or whenever the employer has reason to believe that there are deviations from or inadequacies in the employee's knowledge or use of the energy control procedures. On or about May 3, 2023, the employer failed to retrain all employees involved in a group servicing activity on the Doser machine that was not de-energized prior to performing work. Energy controls were not followed resulting in an amputation to one of the employees. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
