29 CFR 1910.217(c)(3)(iv)(b): Point of Operation Devices. Attachments shall be adjusted to prevent the operator from reaching into the point of operation or to withdraw the operator's hands from the point of operation before the dies close. a) On or about January 19, 2023, the employer did not ensure that the pull back attachments were adjusted to withdraw the operator's hands from the point of operation before the dies close on the Walsh Press & Die Co No 55 full revolution press in that the press's pull out device were not set for the operator and did not pull the operator's hands out before the die closed.
