29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). At location 450 W. 33rd Ave. Oshkosh, WI: the employer did not ensure that employees performing maintenance tasks and or servicing machines on the FLOW 1 and FLOW 2 machines, had isolated all energy sources, including but not limited to pneumatic, hydraulic, and natural gas energy, exposing employees to hazardous energies. Employees used the main control panel touchscreen and hit the system auto shutdown that was used in place of an energy isolating device while performing the work. a) While performing servicing or maintenance work on FLOW 1, an employee did not control the hazardous energies, such as but not limited to electrical, hydraulic, pneumatic, and natural gas. b) While performing servicing or maintenance work on FLOW 2, an employee did not control the hazardous energies, such as but not limited to electrical, hydraulic, pneumatic, and natural gas.
