29 CFR 1910.119(f)(1)(i)(G):The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and did not address the startup following a turnaround, or after an emergency shutdown. a. Machine Room - On or about December 14, 2021, the employer failed to develop specific procedures for start up following a turnaround or after an emergency shutdown for the high pressure receivers that are used in the process of handling anhydrous ammonia, failure to establish accurate safe operating procedures for the ammonia system can lead to process upsets, and expose employees to fire, explosion and toxic hazards. b. Machine Room - On or about December 14, 2021, the employer failed to develop specific procedures for start up following a turnaround or after an emergency shutdown for the intercoolers, that are used in the process of handling anhydrous ammonia, failure to establish accurate safe operating procedures for the ammonia system can lead to process upsets, and expose employees to fire, explosion and toxic hazards. c. Machine Room - On or about December 14, 2021, the employer failed to develop specific procedures for start up following a turnaround or after an emergency shutdown for the evaporators that are used in the process of handling anhydrous ammonia, failure to establish accurate safe operating procedures for the ammonia system can lead to process upsets, and expose employees to fire, explosion and toxic hazards. d. Machine Room - On or about December 14, 2021, the employer failed to develop specific procedures for start up following a turnaround or after an emergency shutdown for the Screw Compressors that are used in the process of handling anhydrous ammonia, failure to establish accurate safe operating procedures for the ammonia system can lead to process upsets, and expose employees to fire, explosion and toxic hazards. e. Machine Room - On or about December 14, 2021, the employer failed to develop specific procedures for start up following a turnaround or after an emergency shutdown for the Vilter Compressors that are used in the process of handling anhydrous ammonia, failure to establish accurate safe operating procedures for the ammonia system can lead to process upsets, and expose employees to fire, explosion and toxic hazards. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
