29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Dick's Sporting Good, Inc. at 31 W Flatiron Crossing Drive in Broomfield, CO: On or before March 30, 2021, the employer did not establish a program consisting of use of locks/tags to prevent the unexpected release of hazardous energy of the trash compactor. Operations employees regularly entered the compactor while it was energized to perform servicing, specifically to unjam material that became stuck in the equipment. This condition exposed employees to crushing hazards.
