29 CFR 1910.23(d)(4): The employer did not ensure that the side rails of through or sidestep ladders extend at least 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder. On or about March 11, 2021 and at times prior, at a work platform of the metal sorting line where an inclined trash conveyor passed, the employer did not ensure the right side rail of the step through fixed ladder extended at least 42 inches above the work platform. Employees were exposed to falls from the fixed ladder.
