29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service: (a) K&B Solutions Warehouse: On or about 02/16/2021, the employer did not ensure that industrial trucks were examined before being placed in service. Employees operated a Mitsubishi fork truck (Model #:FGCZ5N) with a horn that was inoperable. The powered industrial trucks was not inspected before being placed into service. This condition exposed employees to a struck-by hazards. (b) K&B Solutions Warehouse: On or about 02/16/2021, the employer did not ensure that industrial trucks were examined before being placed in service. Employees operated a Mitsubishi EOP Order Picker Forklift with a horn that was inoperable and strobe light which was inoperable. The powered industrial trucks was not inspected before being placed into service. This condition exposed employees to a struck-by hazards.
