29 CFR 1910.119(j)(5): The employer shall correct deficiencies in equipment that are outside acceptable limits (defined by the process safety information in paragraph (d) of this section) before further use or in a safe and timely manner when necessary means are taken to assure safe operation: At a facility located at 802 South Street, Plymouth, WI 53073; the employer did not document or further inspect that the deficiencies reported in the "Mechanical Integrity Piping Inspection" conducted by Gamma Graphics Services, LLC dated August of 2009 had been corrected or that necessary means were taken to assure safe operation when pipes were found to have experienced wall thinning.
