29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: Russell T Bundy Associates, Inc. - Mansfield, OH: On or about February 11, 2020, the employer did not ensure that employees who performed smelting operations were provided with and wore head protection to protect against burn injury from molten metal splash. Employees were exposed to burn injury when pouring and tending the furnace.
