29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination was not made available within 10 working days of initial assignment to all employee(s) with occupational exposure: COMPANY-WIDE: On or about and at times prior to February 11, 2019, the employer did not ensure that employees who signed consent forms indicating their desire to receive the Hepatitis B vaccination series did receive the vaccines. Four employees had signed consent forms requesting to receive the vaccination series. Three of them received the first shot in the series but not the final two; one did not receive any vaccines.
