29 CFR 1926.1207(a): The employer did not provide training to each employee whose work is regulated by this standard, at no cost to the employee, and did not ensure that the employee possessed the understanding, knowledge, and skills necessary for the safe performance of duties assigned under this standard. On or about May 4, 2018, the employer did not provide confined space training to an employee who was required to work in an elevator shaft to perform firewall installation work activities. This exposed the employee to hazardous conditions such as, but not limited to, walking/working surfaces and fall protection.
