29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. On or about March 8, 2018, employees were exposed to struck-by hazards when the machine specific LOTO procedure for Viking Bagger #6 included energy sources and lockout locations that were not included in the step-by-step lockout instructions. Employees prepare the equipment to run at the start of each shift. Employee change film and clear jams during the shift. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
