29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): Work benches in garage, Foley Transportation, 235 Taylor St., Springfield, Massachusetts; On and before November 7, 2017 employees were not inspecting flexible cords prior to use. The use of damaged flexible cords exposes employees to electrical shock hazards from the 120 volt sources. a. One flexible cord was run under the dirt and gravel floor from the West wall to the workbench where it was exposed to vehicle and pedestrian traffic. b. One cord had multiple locations that were damaged and covered with electrical tape. c. Two homemade extension cords which used junction boxes to hold the outlets, had missing knockouts on the boxes.
