29 CFR 1910.178(l)(3)(ii): The employer did not ensure that each powered industrial truck operator completed initial training in workplace-related topics including, (I) Other unique or potentially hazardous environmental conditions in the workplace that could affect safe operation: 918 N. Porter St. in Streator, IL 61364, warehouse, silo no. 2, powered industrial trucks (counter-balanced propane-gas powered forklifts), personnel-carrying work platform: on or about August 2, 2017, forklift operators were assigned to elevate personnel in an unapproved work platform (unique workplace condition) for connecting product transfer hoses and rinsing the silo's interior for product changeovers. Operators did not receive adequate initial training in formal topics and practical exercises pertaining to safely elevating personnel in the work platform while operating truck and its lifting controls. Employees elevated in the work platform were exposed to fall hazards up to 13 feet likely to result in serious injury, and other caught-in hazards from moving forklift parts (mast, lifting carriage) likely to result in fingers/hands amputation. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
