29 CFR 1910.217(d)(9)(iv): Safety blocks were not used when mechanical power press die(s) were being adjusted or repaired in the press: (a) Perry Tool & Die Co. at 11606 Reed Ct., Broomfield, CO 80020: On and before June 23, 2017 the employer did not provide and enforce the use of safety blocks for use whenever dies are being adjusted or repaired in mechanical power presses. Employees operate the Perkins Machine Company mechanical punch press model 300-B in the shop. This condition exposed employees to amputation hazards when operating the mechanical power press.
