29 CFR 1910.178(n)(7)(i): Loaded industrial truck(s) ascended or descended grades over 10% with the load downgrade: On or about April 21, 2017, and in times prior thereto, workers were lifted in a basket on a Sky Trak that was traveling laterally to a slope of approximately 17% with the boom elevated, exposing workers to a fall hazard while repairing and replacing worn portions of the rappelling tower.
