29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a) 312 Old Goff Mountain Road: On or about October 11, 2016 and times prior, the employees were not trained to recognize hazards associated with scaffolds, such as working at heights of up to 11 feet with no fall protection, working levels of scaffold not fully planked, cross bracing missing, and working on mobile scaffold without locking the wheels.
