Site- The employer did not address consequences of failure of engineering and administrative controls regarding mechanical integrity hazards including, but not limited to, the release of LPG and associated fires/explosions due to corrosion of process piping and equipment, failure of process piping supports, failure of pressure relief devices, failure of the flare system, and exceeding the maximum allowable work pressure of low-pressure storage tanks and pressure vessels.
