29 CFR 1910.27(c)(4): The employer did not install fixed ladders with at least 7 inches of clearance from the centerline of the rung to the nearest permanent object in the back of the ladder. (a)Cyclone Rig #17, 05-045-22491-00, Parachute, CO, 81635: On and before October 21st, 2014, the employer did not ensure the fixed ladder had at least 7 inches of clearance from the centerline of the rung to the nearest permanent object in the back of the ladder. The ladder consisted of 1-1/2 inch angle iron which used as rungs to access the upper portion of each "A" leg. Each piece of angle iron was welded directly to the derrick "A" legs. The condition exposed employees to falls, cuts, lacerations, and bruises, while climbing the two ladders.
