29 CFR 1910.27(d)(3): The side rails and rungs of side step ladders did not continue a minimum of 3 1/2 feet above the landing platform: On or about October, 8, 2014, and at times prior thereto, the employer did not ensure that the fixed ladder installed for use to access the bag house platform was constructed so the side rails extended 3 1/2 feet above the landing, exposing employees to a fall hazard.
