29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: (a) On or about 9/22/14 - at the "X" furnace, the "Lockout of "X" Furnace for Reline" procedure no. LQXF0015 dated 7/2/2013 does not clearly and specifically outline the requirements for testing the system to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. (b) On or about 9/22/14 - at the Flux Building for X & U furnaces, the "Changing Claudius Peter Valve Cassette" SJP procedure number QBMA0094 dated 05/15/2012 does not clearly and specifically outline the requirements for testing the system to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. (c) On or about 9/22/14 - at the Water Treatment Plant, the energy control procedures "Lock-Out - Filter Press System" Safe Job Procedure number UM0054 dated 11/08/2013 and the "Changing Filter Clothes of Filter Press" procedure number WT001 dated 01/13/14 did not clearly and specifically outline the requirements for testing the system to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures for the north hydraulic sludge pump.
