29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about August 27,2014 in the Chucker area against the wall, the Sears grinder was used by the employees to remove burrs from drill bits. Employees were exposed to struck-by hazard when the tongue guards were not properly adjusted. The opening on the tongue guard was approximately 3/4 of an inch.
