29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were de-energized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): On or about 13 June 2014 At Naval Base Ventura, Point Mugu, an employee working on the de-energized low side of a 150KVA 4160-120/208V 3 Phase delta/wye dry pack transformer, reached in through the high voltage side which had not been locked out resulting in employee being electrocuted.
