29 CFR 1926.502(i)(4): All covers shall be color coded or they shall be marked with the word "HOLE" or "COVER" to provide warning of the hazard. a. At the above addressed jobsite, on the roof, hole covers were not marked with the word "HOLE" or "COVER" to provide warning of the hazard, thereby exposing employees to falls. This most recently occurred on June 7, 2014, when an employee picked up a unmarked sheet of plywood, stepped forward and fell through the roof floor hole approximately thirty one (31) feet and seven (7) inches. Employee was pronounced deceased at the scene of the accident.
