29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees: (a) Line employees performing a product line change had not been designated as authorized employees and removed scrap from the back of the little shear on the 72 inch Braner Slitting Line, without first affixing lockout devices to the energy isolating devices, thereby exposing them to the unexpected startup of the shear.
