29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Employee who were lowering IVI Dust Collector were exposed to fall hazards in excess of 30 feet when the guardrail was removed to facilitate the lowering of the unit and the employer did not ensure employees were protected from falling.
