29 CFR 1910.1048(n)(3)(ii)(A): The training program did not include a description of the potential health hazards associated with exposure to formaldehyde and a description of the signs and symptoms of exposure to formaldehyde: a) At the Establishment - On or about November 7, 2012, the employer had not provided employees with effective information and training on the physical and health hazards associated with formaldehyde exposure, nor the signs and symptoms of exposure. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
