29 CFR 1910.119(l)(5): A change covered 29 CFR 1910.119(l) resulted in a change in the operating procedures or practices required by 29 CFR 1910.119(f), and such procedures or practices were not updated: At the establishment, the employer did not update current operating procedures and practices related to the installation and intended use of the emergency shutdown control switches located on the outside of the two engine rooms. All of the current emergency shutdown procedures within the SOP such as but not limited to the Frick Ammonia Compressor #1 Fab did not reflect any discussion about the emergency shutdown control switches. Employees were exposed to catastrophic release of ammonia which result in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
