29 CFR 1910.335(a)(1)(v):Employees did not wear protective equipment for the eyes or face wherever there was danger of injury to the eyes or face from electric arcs or flashes or from flying objects resulting from electrical explosion. a.) Ellwood City Forge Company; 800 Commercial Avenue Ellwood City, PA 16117, on or about 08/24/2025, and times prior thereto: Employees were working inside the General Electric 4160 volts T2/T3 switchgear in the Millwright Shop Room. The switchgear was not de-energized prior to performing work operations. Employees were not wearing an arc flash hood and/or face shield while working inside the established working distance in the energized 4160 volt switchgear. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
