Per ISA: Grouped w/ Citation ID: 1-3 29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized personnel from attempting a rescue. On or about 25 June 2025, Gilster-Mary Lee Corporation, located at 615 Old St. Marys Road in Perryville, Missouri, failed to establish emergency response and rescue procedures prior to entry into a known permit-required confined space. An employee entered Sugar Coat 1 Oven to perform scheduled cleaning: a. The employer failed to prevent unauthorized personnel rescue attempts when an employee entered a confined permit space following an incident to attempt rescue; b. The employer failed to verify the ability of the local fire department to perform permit required confined space rescue in the facility's approximately 103 known permit required confined spaces.
