Text For Citation: 01 Item/Group: 003B Hazard:
29 CFR 1926.1053(b)(16):Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired. a. Located at or near 6050 East Broad Street, Columbus Ohio, 43213: On or about April 30, 2025, the employer did not ensure that ladders with visible defects were removed from service and or marked/tagged to indicate that they were not to be used. Employees were using this ladder to enter/exit a trench that had bent and missing rungs. These conditions exposed the employees to falls of up to 12 feet.
